NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital, but who have complex ongoing healthcare needs

Midlands and Lancashire CSU is responsible for the decision making and commissioning processes, working closely with the Local Authority’s, on behalf of Derbyshire CCG’s.

We are currently expanding our service and are looking for a number of motivated and enthusiastic nurses with experience in General Nursing (RGN), Mental Health Nursing (RMN) and Learning Disability (RNLD) Nursing The role involves; assessing eligibility criteria under the NHS National Framework for Continuing Healthcare and Funded Nursing Care 2009 (revised 2012), planning and commissioning packages of care (including Personal Health Budgets), supporting individuals, their families and health professionals through the Continuing Healthcare process (including transition from child to adult services) and reviewing eligibility/packages of care.

Successful candidates will have excellent communication and assessment skills; be resilient, able to work autonomously but also work as part of a team. This is a community based post and although the role is based in Chesterfield, post holders will be expected to be able to travel covering the whole of Derbyshire, and occasionally out of the county.
