An exciting opportunity has arisen to join our forward thinking diverse Countywide Out of Hours nursing service. Although this is a post within the County wide Team it will primarily be based within the South of Worcestershire based at Worcester, but there may be occasions when the post holder will be asked to cover teams in the North of the County.

The Team provides specialist nursing care to patients in their own homes, to aid prevention of acute admission, and to support early discharge. It is an exciting time in the community setting, with the role out of Neighbourhood teams, that incorporate Enhanced care and District Nursing, and the Out of Hours Team in South Worcestershire are a paramount part of those Neighbourhood teams across the County.

You should be highly motivated and be able to deliver high quality evidence based nursing care within the community setting to support those who wish to stay at home and avoid hospital admission. You will have good communication skills, be able to work autonomously but also work in an integrated way. Close working with our Neighbourhood teams, Care home practitioners and social service colleagues is essential to ensure seamless care to our patient group.

You should be a good team player yet autonomous in your care delivery as this post requires an ability to be flexible and ‘think on your feet’. Applicants should be fully qualified and hold current registration with the NMC.

We can consider the option for developmental posts for those wishing to progress to the next step in their career; a stepped support approach for those wishing to return to practice following a career break or a senior role for those with experience wishing to broaden their clinical remit.

You can expect personal and professional development including a comprehensive in-service training programme, support from senior nurses and a structure offering career mobility and mentoring. In addition, there is an exciting opportunity to contribute and plan for the future development of community services across the sustainability and transformation programme.

This is an exciting opportunity for a nurse who wants to be at the forefront of integration and care closer to home. You would need to meet the travel requirements of the post including transporting equipment in your car.

Due to the nature of the post the applicant must be able to travel independently and have access to a suitable vehicle for business purposes. Whilst the base is identified, staff can be asked to work anywhere within the Business Unit or Service if necessary.

The Trust offers a preceptorship year for newly qualified nurses and AHP's and attendance at preceptorship programmes is a condition of employment.


Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.


Tier 2 Certificate of Sponsorship
