Leeds MindMate SPA is a Leeds Community Healthcare hosted service that receives referrals for school age children and young people, who need support with their emotional wellbeing or mental health.

As part of the ongoing development of the service we are recruiting to a brand new role of a Triage Support Practitioner. Key functions of the role will be to take telephone self- referrals from families and young people. Follow up electronic self- referrals. Assist with follow up calls to professionals and triage specific cases as allocated. You would also support the customer service feedback process.

The Service operates Monday - Friday with phone lines open between 9-5, the team comprises of administrative staff and clinicians from services such as The Market Place and CAMHS.

The role will require excellent people and communication skills with a confident telephone manner. You will need the skills to take self-referrals, responding appropriately to families and young people. You will also need the ability to liaise with professionals from a variety of disciplines.

Experience of working with young people, mental health services and risk management is also key. Knowledge and/or experience of neurodevelopmental issues such as autism/ADHD and an understanding of young people's mental health services across Leeds would be an advantage.

We work across 4 databases (Mosaic, CareNotes, Systm1 and Synergy), so good IT skills are essential with the ability to accurately take notes while on the phone.

Applications are welcomed from clinician with experience of working within the Leeds Cluster model, CAMHS practitioners or other professionals with experience of working with Children and young people to support emotional wellbeing and mental health needs. You must have a relevant professional qualification and professional registration.

We would also welcome applications from clinicians considering a secondment to this service/role.

Leeds MindMate SPA is based in Leeds City Centre at Merrion House with state-of-the-art facilities and easy access to public transport. Please be aware that we are unable to provide parking.

If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost (Enhanced £48.83 or Standard £30) from your salary. There will be no charge if you are enrolled in the DBS Update Service.
