Six-month fixed term

An exciting opportunity to join our team as a Continuing Healthcare Nurse Assessor required to provide specialist CHC/FNC nursing assessment, review and case management of adults and older people residing in some acute settings, care homes with nursing, residential care settings and individuals within their own homes in line with the National Framework NHS Continuing Healthcare and the determination of the Funded Nursing Contribution (FNC).

To work closely with local partners, to include health, social care and independent providers in the planning, commissioning of care, management and review of patient care needs. Provide clinical advice in relation to NHS Continuing Care and NHS Funded Nursing Care (FNC).

To provide assurance around quality of commissioned care packages, responding to and reporting safeguarding concerns in accordance with agreed interagency reporting procedures.

Pay and conditions

The national pay system for the NHS — Agenda for Change (AfC) — is for all directly employed staff except doctors and the most senior managers.
