Tower Hamlets Home Treatment Team (TH HTT) are seeking to recruit a highly skilled and experienced mental health practitioner to join the Tower Hamlets Home Treatment Team as Band 6 Community Mental Health Nurse Team serving the needs of people within a diverse population.
The HTT provides a safe and realistic alternative to hospital admission in familiar surroundings in order to aid individuals recovery during a period of mental health crisis. TH HTT is HTAS accredited and recognised for its good work with Quality Initiatives.

 

The TH HTT works successfully to provide a true alternative to hospital admission and works in close collaboration with the inpatient and community teams as well as the Emergency department and the voluntary sector crisis house. The crisis pathway in Tower Hamlets will continue to develop in offering a wider range of crisis services.

 

The post requires clinicians to be confident assessors who can make clear decisions about risk assessment, management, care planning and the appropriateness of home treatment team interventions. Effective gatekeeping assessment and the identification and delivery of the best treatment packages for people presenting to services in crisis will also be key in addition to support of relatives and carers.
