Community Healthcare Assistant

Band: 3

Salary: £18,813-£20,795

Hours: 37.5 per week to include 1 late shift per week and weekends on rota

Base: North Cotswolds – Bourton on the Water & Moreton in Marsh

Gloucestershire Care Services NHS Trust provides care for approximately 600,000 adults, children, and young people across Gloucestershire, including community inpatient and outpatient services, public health nursing, community nursing and therapy staff.

We are seeking to recruit a dynamic and motivated Health care assistant to work in the North Cotswolds. You will work alongside the nursing team as part of the wider integrated team, delivering healthcare to patients in their own homes.

The post offers the successful candidate the opportunity to work in a community nursing environment developing a proactive approach to meeting the health needs of the local population as part of the integrated health and social care community team.

Gloucestershire Care Services are already delivering Integrated care, and were doing so ahead of the recent Government direction towards this model of care. This is an exciting opportunity to join a forward thinking and dynamic organisation.

Applicants should have excellent communication skills, a passion for learning and an ability to take a proactive approach to meet the needs of the local population.

We aim to provide high quality care and work closely with the acute trusts, GPs, our colleagues within integrated community teams, Urgent and Unscheduled care services and other local agencies to ensure that our patients receive a high quality service.

Professional development is available. We offer a fully supported induction programme, annual appraisal and personal development plans. We also offer access to a variety of in house education and training programmes.

The ability to travel in the community and hold a current UK driving licence is essential.
