We are looking to recruit a dynamic Case Manager to support the delivery of community nursing to people in their own homes within the Holderness Locality.

You will manage and be responsible for a defined caseload of community adult clients with multiple and complex needs, using evidence based/client centred principles to assess, plan, implement and evaluate interventions delivered by your team.

The post requires excellent communication skills, the ability to show flexibility with duties and have strong leadership, decision making skills and clinical expertise. The ability to work flexible hours including weekends and bank holidays is also required.

To succeed in this role you will need to be passionate about community healthcare provision, able to tackle key priorities with enthusiasm and have the tenacity to deliver in an ever changing environment.
