We are looking for enthusiastic and motivated experienced rehabilitation assistants to join our highly regarded CSH Surrey Rapid Response Service and can offer either full or part time hours.
Based on the St Peters Acute Hospital site just of Jct 11 of the M25, we provide responsive community services to patients across the beautiful Thames and surrounding regions. We also provide an In Reach service to Accident & Emergency supporting acute hospital admission avoidance.

Working in our Multi-Professional team you will provide therapy support to the clinicians and provide prescribed rehabilitation packages of care to support patients in their own environment, preventing unnecessary acute hospital admissions and supporting complex discharges from hospital.
Although expected to work autonomously you will not be alone, the team is supported by a clinical service manager, Advanced Nurse Practitioners and Team Leader Physio and Occupational therapists. You will also have the support of GP’s, Rapid Response Matrons and nurses.
In this challenging role you will be expected to have:
• Desire to undertake further training as appropriate
• Strong written and verbal communication skills
• Excellent time management and organisational skills
• Ability to work under pressure and prioritise caseload
• Motivation to develop and extend knowledge / expertise
• Ability to work independently and responsibly, without direct supervision
• Experience of working in a health care environment would be preferable but not essential
• Work in a collaborative and integrated manner with other stakeholders to ensure a seamless service delivery

• Ability to work both independently and also as part of the wider multidisciplinary team
• Ability to relate to patients and colleagues in a sensitive manner

Successful applicants, who have not worked for CSH within the last 12 months, will receive an amount of £2000 paid in 2 instalments - £1000 upon commencement and a further £1000 following successful completion of probation.
