Advanced Nurse Practitioner Frailty

Hours of duty: 37.5 per week

Responsible to: Operations Manager

Accountable to: Care Group Director – Care Group 1


We are looking for an experienced ANP to join our Frailty Team within the East Riding of Yorkshire.


City Health Care Partnership CIC (CHCP) is committed to improving outcomes for our local population, especially those who are severely frail, who are at significant risk of unplanned admissions and for whom hospital is not always a place of safety due to risks of deconditioning and increasing dependency.


This community based post is aimed to support care home residents to maintain their independence and prevent unnecessary hospital admissions. This is achieved with patient centred management plans implemented by the Multidisciplinary Team, involving the patient, their families and carers, including advanced care planning.


The successful applicant with forge links with primary care and existing stakeholders with a focus on the development and implementation of clinical support for the care homes and implementation of diversionary pathways. The post holder will work alongside the named nurse within the care homes to ensure personalised management plans are effective and regularly reviewed for care home residents.
