Clinical Lead Frailty – Luton Community

Band: 7

Hours: 22.5 hours per week with a possibility of increasing to full time.

Contract: Permanent

Base: The Poynt, Luton

 

Is this an opportunity for you?

You will have passion and drive with a strong team ethic and flexible disposition, responding positively to a changing environment, and be willing to progress your own skills and knowledge base and those of others.

You will work in close collaboration with care home providers, GP’s, the integrated communitynursing teams, mental health, social services and the voluntary sector to promote falls prevention. This includes delivering teaching packages to groups of staff both internally and externally. Excellent communication skills both written and verbal are essential.

You will coordinate the team activities so that the team members are self-directed in daily service provision. Provide clinical support, advice and leadership tothe falls teamto ensuredelivery of outstanding care.This includes the development and support of staff to ensure they have the skills and knowledge required.

You must be a Registered Nurse or Allied Health Professional with demonstrable experience and knowledge of falls prevention, frailty and management of these in the community.
