We have two Community Learning Disability Nurses - Band 6 vacancies:

Position 1- 0-25 Together Team Central Team

We have an opportunity for a Band 6 Nurse to work within the 0-25 Together Team providing nursing input to young people aged 18 -25 years with learning disabilities. You will be involved in supporting young people with learning disabilities and their carers to understand and manage the transition from children’s to adults health services, alongside team colleagues supporting their social care needs. You will be working with the wider health partners to address health inequalities for young people with learning disabilities during transition and increase the uptake of GP health initiatives for people with learning disabilities and their carers.

Position 2 - St Albans Adult Disability Team

We also have an opportunity for a Band 6 Nurse to work within the St Albans Adult Disability Team providing nursing input for adults with learning disabilities living in the community to address existing health inequalities alongside team colleagues supporting their social care needs. You will work alongside primary health colleagues through the GP Link Nurse role to ensure that proactive approaches are taken to reduce the health risks for people with learning disabilities and advise on reasonable adjustments that can be implemented. You will also have the opportunity to take part in innovative projects designed to improve the understanding of health risks for people with learning disabilities and their carers.

Applicants must be registered as a learning Disability Nurse with a valid NMC registration and experience working in the community. In your application you need to demonstrate your experience in working with people with learning disabilities through examples that show you have the skills and knowledge as outlined in the attached Job Profile.

Due to both these posts being community based the successful candidate will need a Full Driving Licence.
