Adult Care Management Team Chiltern is based in a beautiful market town of Amersham with excellent transport links including London underground.

About the role

 

This a dynamic team working withclientswith physical disabilities as well as older people. We complete assessments and reviews of care plans and develop long term care and support plans.

This is a perfect environment to really foster your social work and analytical skills as we are typically involved with service users requiring longer term support. The nature of work is very diverse so you will never be bored, italsoenables our Social Workers to beable to find the areas of practice which are of interest to you. We work in a strengths based framework meaning that we explore and incorporate clients’ networks of support in their assessment and support planning.

We are committed to supporting staff development through provision of appropriate training and postgraduate qualifications for qualifying staff members.

We are going through exciting transformation aimed at simplifying paperworkin order to empower our staff to utilize theirsocial work skills. If you are an enthusiastic person with good analytical skills who is able to embrace changescome and work with our amazingteam!

About you

You will hold a recognised health and/or social care qualification with2 years plus post qualifying experience. A car user and registration with the HCPC/NMC is essential for this post.

Experience and knowledge required;

Familiar with legislation for adults and be able to apply it to practice

  • You will be proactive in developing your knowledge and skills and sharing them
  • Recent experience in a health or social care setting
  • Excellent assessment, report writing and organisational skills
  • Work in partnership with people with diverse needs, both service users and carers; and colleagues and providers
  • You will be caring and collaborative; motivated and reliable; flexible and resilient
  • Have the ability to organise your own time and be able to work to deadlines
