Care Plus Group are seeking to recruit a Clinical Lead to work within our Hospital Discharge Team. As Clinical Lead you will provide leadership and management to the clinicians within the team, supporting them in the safe, effective and timely discharge of patients from hospital.The Hospital Discharge Team is a joint team staffed by Care Plus Group, FOCUS and NAVIGO Staff, working in partnership to support and enable safe and effective hospital discharge. The service is based in the Operations Centre at Diana Princess of Wales Hospital working closely with the acute hospital setting providing comprehensive health assessment and case management support, to individuals in hospital or Intermediate Tier services or by way of deflection and prevention of admission where appropriate. This post involves 7 days per week working on a rota pattern of early and late shifts.

The post holder will work in partnership with the General Manager - Hospital Discharge Team, providing clinical leadership and management to the clinical staff within the team. You will ensure safe and effective hospital discharge is effected by way of assessment, planning, co-ordination and communication at the interface between hospital and community in order to enable a smooth transfer of care between Secondary and Community Care. Undertaking and supporting colleagues with assessment of Complex Health and Social Care needs is an integral part of the role. The post holder will proactively manage episodes of care to reduce the length of stay in hospital, promoting effective channels of communication between all agencies involved in the continuity of patient care. You will represent Care Plus Group within the Hospital Trust providing appropriate challenge regarding Delayed Transfers of Care and Stranded Patients.

We are looking to appoint a suitably qualified, experienced and enthusiastic Clinical Lead to join The Hospital Discharge Team. You should be:

A Registered General Nurse with a minimum of five years’ experience. You must demonstrate previous experience of working in a community nursing setting with demonstrable Continuous Personal Development. You must be able to demonstrate effective Inter-Agency working and communicate across a wide range of professionals and disciplines as this is a key element of the role. Ability to manage own workload is essential. An understanding of Statutory and Voluntary Service provision is key with previous experience of planning Complex Packages of Care essential. Comprehensive knowledge of End of Life Strategy and Continuing Health Care Framework is key to this role.
