The South Gloucestershire Community Rehabilitation Team (CRT) provides active rehabilitation to patients in their place of residence to maximise their functional potential and support them to remain independent within their homes. We aim to assist in preventing hospital admission and to facilitate timely hospital discharge and to ensure the patient, as well as providing ‘planned care’ to assist patients to remain in their home setting. The CRT is a multidisciplinary team that provides a flexible service working extended hours to support patient needs.

Due to an increase in funding to support expansion of Discharge activities we are seeking an enthusiastic and innovative occupational therapists to provide exceptional therapy intervention to patients in their place of residence. This will include work to facilitate patients to return home from an acute stay, work to prevent admission for a patient in crisis and to undertake domiciliary therapy to prevent decline and functional deterioration.

You will be working within the Kingswood Locality Community Rehabilitation Team, that includes Senior Physiotherapists, Occupational Therapists and Rehabilitation Support Workers and working closely with our Community Nursing colleagues.

You will provide support and guidance to support staff, students and other therapists, so will be required to demonstrate good communication and be motivated to support others in their professional development. You will be supported in your own development by our experienced senior team, with access to regular training, that is both clinical and non-clinical e.g. leadership skills in nature.

Good team working, time management and the ability to take initiative are also required.

Working arrangements include weekends and bank holidays on a rota basis.

Registration with HCPC is required.

Appropriate OT Professional Qualification e.g. BSc or Diploma required.

Evidence of post graduate courses and demonstrable experience in community based rehabilitation.

Valid Driving Licence and access to vehicle
