Are you looking for a new challenge in a community Health and Social Care environment?

This is an excellent opportunity for a suitably experienced and motivated person to work as part of the clinical Therapy Team within the multidisciplinary neighbourhood teams. The position will initially be based at Yeadon Health Centre but may require some flexibility across our teams in the North West of Leeds.

Working as part of an integrated multidisciplinary team you will be required to:

  • Complete basic assessments within your competency and supervising, monitoring and progressing treatment plans set by the neighbourhood team
  • Supporting patients with a focus on rehabilitation
  • Ordering, fitting and teaching the correct use of basic equipment to support with mobility and activities of daily living
  • To take basic observations and understand when to seek advice
  • Liaising with other health and soci al care professionals, patient families and other agencies, to manage risk, maximise quality of life and promote self-management

The service offers structured appraisals and clinical supervision and actively promotes Continuing Professional Development through in service and external training.

Interview dates will potentially be the week commencing 28th January 2019.

About you

You will need to demonstrate excellent communication skills, have previous experience of working in a community healthcare setting and of multi-disciplinary working. You will need the ability to multitask in a busy environment and work under your own initiative, and be able to manage your work load flexibly to meet the demands of the service. An understanding of patient centred care using rehabilitative approaches would be beneficial.

If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost (Enhanced £48.83 or Standard £30) from your salary. There will be no charge if you are enrolled in the DBS Update Service.
