The Jean Bishop Integrated Care Centre is in East Hull and is part of a new service commissioned by NHS Hull CCG. It has been built specially to care for older people in the community and is run by City Health Care Partnership CIC.

Patients will be able to have a comprehensive assessment through an Integrated Care team that includes doctors, nurses, therapists (like physiotherapists and podiatrists), pharmacists and social care teams. This team will discuss all your health, social and personal care needs with you and they will make sure you have a clear plan on how to address these needs. Volunteers will pay a valuable role in improving the patient experience by guiding patients to the ICC gym for cardiac and pulmonary rehab sessions, and being a point of contact for patients/carers in the building.
