We are looking for experienced Community Nurses who are after a new challenge and who want to join our hardworking and dynamic team in Wandsworth.

We aim to ensure that quality is at the heart of everything we do ensuring that we have well resourced, well-structured, thoughtful and efficient services run by staff that care passionately about the patients we serve. The Wandsworth Community Nursing Service provides care across the London Borough of Wandsworth to a diverse population. As the Band 7 District Nursing Team Leader, you will be leading and managing a community nursing team, responsible for the provision of excellent community care to patients in their homes. As part of this, we aim to facilitate self-care for our patients with long term conditions, health promotion and the use of innovative activities and projects to enhance the patient experience.

We are looking for an individual with recent post-registration nursing experience in community nursing and a post–registration qualification in district nursing or equivalent. You should also have experience of negotiating and liaising within a multi-disciplinary team and experience of working with people with complex needs.
