We are looking for dynamic and motivated Band 7 Adult Nurse / Paramedic / AHP (Advanced Assessor or equivalent) to work with us in a new team that will provide a Telemedicine Service across North West London. The Telemedicine service has been created as an innovative, partnership model to provide responsive, caring advice and support through interactive telephone and video consultations to care home staff across North West London. As such, the project aims to support residents in over 130 care homes across the 8 CCGs of the North West London Sustainability and Transformation Partnership. The service will provide an extension to the current 111 service provision across North West London with the aim of allowing more nursing home residents remain at home.

 

We are looking for RNs (Adult) who have an interest in end of life / palliative care to work in the team. You will have excellent clinical skills as well as the ability to work both autonomously and within a team, be able to manage conflicting priorities and have strong communication skills. As a member of our team you will be expected to both support the care home staff and, using the technology provided to complete an assessment, including a provisional diagnosis and implement an initial treatment plan. The team will have access GP records and will be able to access local primary care and other provider systems.

 

The service forms part of West London NHS trusts ambitions to continue to expand the community portfolio. As the project develops we envisage that there will be opportunities for the Telemedicine staff to develop closer clinical links via rotation with the Raid Response Teams.

 

The post holder will be required to work a flexible shift to meet the needs of the service.
