Band 6 Physiotherapist Advert (2.00 WTE)

Due to an internal promotion, the Community Independence Service (CIS) has an opportunity for a Band 6 physiotherapy to join our rehabilitation team, working on falls prevention, frailty management and dementias to name a few.

 

 

 

We are achieving excellent results in community rehabilitation and admission avoidance for the people of Hammersmith and Fulham and are part of a wider community independence service offered in Westminster and the Royal Borough of Kensington & Chelsea. As a band 6 Physiotherapist you will provide an integrated Health & Social Care service for people at home who are at risk of being admitted to hospital unnecessarily, providing Community Rehabilitation, Rapid Response, Reablement and Supported discharge. You will be working closely with our acute partners, such as Imperial College Healthcare NHS Trust and Chelsea & Westminster Hospital NHS Foundation Trust.
