Would you like to be involved with that development and shape a robust community service that supports primary care and also facilitates faster discharges from the acute sector as well?

We are looking to recruit someone who would like to play a key role in delivering patient care, enable and educate GP's, Practice Nurses and Community Nurses. Along with working and interfacing effectively with in acute settings.

We are based in Morden and work within the London Borough of Merton. Morden Underground station is just around the corner and Wimbledon Station 1.5 miles away. We are on many bus routes, with a tram station very nearby.

We have are a multidisciplinary team consisting of Consultants, Dietitians, Podiatrist as well as Specialist Nurses. The team receives referrals from many primary care and community services as well as local acute hospitals. The team meet regularly for clinical supervision and case discussion.

We are a unique community provider as we manage people with both Type 1 and Type 2 diabetes and are an initiating centre for Freestyle Libre. We also deliver DAFNE & DESMOND structured education.

So what do you need to join our team?

    • To be a Registered Nurse.
    • Relevant post registration training in Diabetes e.g.: Masters or diploma in diabetes management.
    • Independent Nurse Prescriber or be able to demonstrate an ability that will enable attendance of a prescribing course within 6 months of starting post.
    • Significant experience of assessing and providing evidence based care to people with Diabetes.
    • DESMOND &/or DAFNE accreditation is desirable or willingness to undertake training for these structured education courses.

Please note that Part-Time & Job Share will also be considered for this position.
