LEICESTERSHIRE PARTNERSHIP NHS TRUST COMMUNITY HEALTH SERVICES MENTAL HEALTH SERVICES FOR OLDER PEOPLE

Band 6 Memory Service Nurse (Mental Health Nurse) 22.5 hours (1 post)

The post will cover East Leicestershire and East Leicester; Base Evington Centre, Leicester General Hospital, and West Leicestershire and West Leicester; Base Bennion Centre, Glenfield Hospital. It will be based predominantly at Neville/Evington Centre.

We require an enthusiastic, motivated and experienced band 6 RMN to join our memory service, providing a caring service to people who have been referred with suspected dementia, supporting with assessment, imparting diagnosis and commenced treatment where relevant. The service will operate within out-patient settings, care homes and clients own homes. Our specialist services works with consultant psychiatrists, operating from a variety of outpatient clinics within East and West Leicestershire CCG, and East and West Leicester City CCG.

You would join an established team of memory service nurses who operate across Leicester City and Leicestershire, (based within East locality predominantly), however applicants may be called upon to cover the whole service. You will need to demonstrate flexibility and a commitment to high standards of patient care. You should also demonstrate good assessment and communications skills and the ability to work alone and as part of a team.

Possession of the non - medical prescribing qualification or willingness to work towards this is valuable to this role, as you will be monitoring and titrating medication in line with national and local treatment protocols. The team has close partnerships with CMHT’s and other health professionals within the wider context of Community Health Services, Social Services and the voluntary sector to ensure the patient experience is maximised. We are looking for a candidate who is motivated and enthusiastic to achieve this.

We have a strong commitment to supervisory training and PDP, from which you will benefit and provide to other team members. An ability to be able to access a wide range of community locations without relying on public transport is essential and a car-leasing scheme is in operation.
