This role is an exciting opportunity to work as part of the Learning Disabilities service in Lincolnshire. The Learning Disabilities Physical Health Liaison Nurse post is part of the multidisciplinary team in the East of the county. The post involves managing a caseload of people with learning disabilities who also experience complex health needs that cannot be met without the support of specialist services. This will involve working with the wider multidisciplinary team and social care community but also working closely with the acute liaison nurses to support individuals accessing primary and secondary health services to ensure reasonable adjustments are considered in hospital but also in the community setting.

We are looking for a motivated RNLD Nurse (or RMN, RGN) with experience of working with adults with learning disabilities and complex health needs. It involves working across LPFT and with the wider health community in partnership to ensure people with learning disabilities receive equitable access to services to have their health needs met.

You need to have an extensive knowledge and understanding of the physical health needs of people with learning disabilities, the safeguarding procedure, The Mental Capacity Act and the Equality Act to enable you to advocate for people with learning disabilities.

The role requires the ability to manage a complex caseload but also prioritise the GP practices in your area to ensure their learning disability registers are up to date and reflective of the population. You will also be expected to provide teaching sessions on a formal and informal basis to your GP practices around the health risks that people with learning disabilities experience and liaise with them as required. Whilst this post is based in the East you will be part of a wider physical health team within the learning disabilities service providing support to colleagues if required.

You will also work as part of a well-established multi-disciplinary team and you must be a compassionate, organised, team player with excellent communication skills and effective time management skills. You will ideally have experience in the widely recognised health needs that people with learning disabilities experience and be able to understand the implications this can have on their presenting condition. You need to have the ability to advocate on behalf of people with learning disabilities to ensure their health needs are understood and met having the confidence to challenge the decision of others in a safe and supportive manner.

The successful post holder will receive regular supervision both management and clinical and there are future training opportunities available.

Core hours of work are Monday to Friday 9am to 5pm (but there may be a need for flexibility to meet patient need on occasions). Part of the role will be to meet with other physical health colleagues from the service as required, but also attend regional forums so the applicant will be required to be able to travel independently to areas.
