The Surrey Heartlands Health and Care Partnership is focused on how the NHS and local government can work together to improve and join-up services to meet the changing needs of all of the people who live in our area. We intend to do more to prevent illness, support people to manage existing health conditions and to stay independent. To do this we are developing seamless health and social care services.

The partnership presents a new way of working. It ensures that all the health and care organisations in Surrey are working together in a join-up way. With a focus on prevention and support that’s targeted where it’s most needed, we will reduce the unfairness some people experience in accessing care, so nobody is left behind. At the same time, we want to take advantage of what we have in Surrey to pursue innovation with business, public sector partners and communities, joining up services for residents and developing digital technologies to create smarter ways of managing health and accessing support.

Surrey Heartlands Health and Care Partnership has established a Transformation Unit that will support Innovation, Transformation and Improvement across the system. The Transformation Unit is a permanent Programme Management Team that supports the mobilisation and coordination of change across the system. The Transformation Unit acts as the system’s own internal consultancy and can be ‘flexed up’ and ‘flexed down’ to meet the needs of the ICS, Place, and the Provider Collaborative. The Transformation Unit will adapt generic project, programme and change management skills to meet the requirements of the system, with a focus on benefits realisation and the transfer of project and programmes to mainstream provision once the aims of the project and/or programme have been achieved. The Transformation Unit brings together the right tools and people acting as a catalyst for the system to develop innovative solutions to some of our most ‘wicked problems’. The Unit will support our leaders to deliver the solutions our system and population deserve

The ICS Transformation Programme Manager will be responsible for the management and successful delivery of programmes of strategic importance to key clients and stakeholders on behalf of the system. Key clients and stakeholders will include Place based leaders, the Provider Collaborative, and the Integrated Care System itself. They will have overall responsibility for all projects within their programmes and will be responsible for building and strengthening strategic programme relationships with clients and stakeholders. The management of relationships will also be supported by other members of the Transformation Unit Senior Management Team, however overall responsibility will remain with the Programme Manager.

The Programme Manager will develop effective work programmes to enable project planning, delivery, and progress reporting. Working closely with Programme Management Office and Senior Management Team, the Programme Manager will identify and lead opportunities preparing business cases and proposals as required.
