Secondment Opportunity for 2 x Band 4 Nursing Associate or Associate Practitioner roles – Regaining Independence Community Day Unit Service (Internal Only)

An excellent opportunity has arisen to join The Community Day Unit Service currently based at The Gouldings in Freshwater.

We are looking for an individual for this varied and developing role who require excellent interpersonal skills and a passion for learning and development in the field of reablement and rehabilitation. You will need to demonstrate flexibility, reliability and excellent organisational skills. Highly motivated, you will enjoy working as part of a dynamic and developing MDT providing holistic, person centred care in a community setting.

The successful applicant will be the point of contact for the service, signposting assessment and prescription of programs of rehabilitation and reablement, spanning Physiotherapy, Occupational therapy, Speech and Language therapy and Nursing. This can include optimising mobility, re-training personal and domestic activities of daily living, building confidence and providing appropriate multi-disciplinary input and guidance.

In return for your skills and commitment we offer regular supervision and support from qualified health care professionals, training tailored to your role and learning needs. You will benefit from working with experienced and supportive colleagues from a variety of clinical backgrounds and be able to develop your skills in working with adults requiring rehabilitation and reablement services.

The posts would suit applicants with experience of working in health or social care setting at Band 3 level who wish to develop their skills and knowledge in the rehabilitation setting.

This post will involve rotating between services within the west and central Locality supporting a 7-day service. A car driver is essential. Hours are 08.30 - 16.30. The post is for 37.5 hours per week.

It is expected that you will have achieved NVQ3 in Health and Social Care or equivalent plus, additional training and experience of working in a relevant health or social care setting including knowledge of nursing/physiotherapy/occupational therapy/speech and language therapy.
