Bolton NHS Foundation Trust was formed in 2008 and, in 2011, we became an integrated provider with community health services joining the trust. Covering a catchment area that also incorporates parts of Bury, Wigan and Salford, we deliver hospital and community based services from the Royal Bolton Hospital in Farnworth and from 20 health centres across Bolton, to a catchment population of 320,000 people. We have a CQC rating as good overall and we have some key areas of outstanding practice with an outstanding rating for being well led at every level.

Bolton NHS Foundation Trust is an integrated provider of acute, intermediate and community health care serving the population of Bolton. We are seeking to appoint enthusiastic, motivated, forward thinking Band 4 Qualified Nursing Associates (or those due to qualify this year) to work within the community working within our district health and care teams, providing home-based proactive and reactive care. The role will work with a range of health and care professionals and will be supervised by our nurses. The role includes supporting our newly developed Covid oximetry at home service. This service is part of the Integrated Community Services Care Division who provide a multitude of services within Bolton. Experience of working in a community setting is not essential as the successful candidates will receive a full supportive induction when they join the Division and the team.

The post involves 7 day working which includes bank holidays.
