Exceptional PA opportunity – with opportunity to rotate

We’re seeking dynamic, empathetic & enthusiastic Physician Associates to join Solent NHS Trust, working within our Adult Mental Health team and Adult Intermediate Care services.

An innovative and varied role, ideal for a PA who wants to try something different in their career with two different aspects to the role. Firstly, we are offering experience in an Adult Mental Health setting where you will be taking the lead for making sure our clients have their physical health needs met. The second role is based in our Intermediate Care Service where our multidisciplinary team works to promote independence of frail adults by reducing hospital admission and facilitating faster hospital discharge; the role will be based in our adult rehabilitation wards initially. We can offer the roles as a rotation or as part of an agreed weekly split role – we can be flexible for the right people!

Working autonomously but with appropriate support, you will provide care for the presenting patient from history taking and clinical assessment through to the diagnosis, treatment, and evaluation of care. You’ll demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe care for all patients.

You’ll be a graduate of an accredited UK Physician Associate training programme and have Registration with the UK Managed voluntary register.

Summary –Adults Portsmouth Inpatient Rehabilitation wards provide general rehabilitation services to residents of Portsmouth aged over 18. The wards help people recover from acute illness and injury and restore mobility and function so they can return home. In addition, the wards can take people struggling at home as a step-up admission providing an alternative to acute hospital admission. Multidisciplinary team working and comprehensive assessments are key and there are strong links to community teams, primary care and to the acute trust. Medical support to the wards is provided through weekly consultant-led ward round & MDT meeting. Further medical support is provided through AHPs, middle-grade doctors and GPs depending on the acuity of the patients on the ward.

The inpatient wards are part of the Intermediate Care Service within Solent Adult Services Portsmouth. Intermediate Care also encompasses the Discharge Hub which facilitates patient flow out of the acute hospital and the Portsmouth Rehabilitation and Reablement Team (PRRT) which supports people in their own homes. In addition, the Intermediate Care Service works closely with Portsmouth City Council Discharge to Assess & rehabilitation bedded unit at Harry Sotnik House, Portsmouth.

Summary –Adult Mental Healthteam provides care for working age adults from the Portsmouth area. Within Solent the closest working relationships are likely to be with the established wellbeing team, the psychiatrists in the team, and the nurse prescribers. Externally the target is to more fully work with community GP to ensure strong collaborative shared care.

The patients we care for have severe and enduring illness. The culture of the Trust is that people in a phase of improvement and respite are best cared for by GP rather than “monitored” in secondary care. Care is delivered on site in St Marys Hospital where both traditional appointments or remote working with patients will be based. In exceptional circumstance some domiciliary visits may occur, but these are very rare.

In a developing service it may be that some remote working/working from home couldbe part of this role and this will be discussed at first job planning. Our target is to deliver holistic health care and avoid the distinct “Mental health” and “Physical health“ boundaries. This service is designed to reinforce this model and is based on review of current health, assessment of needs and intervention, possibly supported by other specialties.

Job Responsibilities

You will provide care for the presenting patient from history taking and clinical assessment through to the diagnosis, treatment, and evaluation of care together with liaison with family members and other health and care services on the ward. You will demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe care for all patients. You’ll work collaboratively with the Intermediate Care Team and Mental Health team to meet the needs of the patients, supporting the delivery of policy and procedures and participating in weekly MDT Meetings.

You’ll provide a holistic clinical service, with support from therapists, doctors and service line managers to implement agreed management plans and follow approved protocols and clinical governance procedures as appropriate. Over time it is hoped that you’ll contribute to service development and evaluation.

Mentorship and supervision will be provided by designated medical supervisor, with teaching and support from them and their nominees/delegates. The level and type of supervision will be dependent on your acquisition of skills and knowledge and determined by the organisation’s clinical governance arrangements. The Trusts holds a variety of academic and training meetings and the PA will be expected to attend and partake in these at the direction of their medical supervisor.
