SOCIAL PRESCRIBER

Hours: 37 Hours Per Week

Salary: £23,057 - £24,830 Per Annum

Location: GP Practices within the Eastleigh Network

Solent Mind is working with the Eastleigh Primary Care Network to deliver the social prescribing service across the network of local GP Practices.

The Social Prescriber will work as part of the multi-disciplinary health care team, providing support to people to improve their health and wellbeing, helping them live more independently and ensuring they are able to access resources within their local communities. The approach will be person-centred, with a focus on strengthening both community and personal resilience.

There is a possibility that a higher salary can be offered within the range for the right candidate.
