This is an exciting and unique opportunity to work as a band 4 assistant practitioner with Community Rehab Team. The post is based at Exmouth Community Hospital providing rehabilitation for people in their homes across Woodbury, Exmouth and Budleigh (WEB).

We are looking for X 2 full time candidates, who can demonstrate a person- centered approach, interested in working as part of an interdisciplinary team and who is keen to develop their rehabilitative therapy skills.

As a band 4 you will be expected to progress and take responsibility for progressing patients with non-complex needs under the direction of a registered health professional.

You will be part of a multi professional team, providing a high quality clinical and support to people in a crisis situation in order for them to remain in their own homes . You will be involved in all aspects of the assessment, treatment and discharge process, and work alongside both health and social care colleagues to ensure that patients remain as independent as possible and achieve the best possible health and social care outcomes.

Main duties of the job

  • To provide specialist therapeutic interventions including personal care under the direction of therapists (physiotherapy, occupational therapy), to patients in their own home/communities, in order to allow patients to achieve their optimal level of independence and maximum rehabilitation potential.
  • To contribute to the assessment, implementation, evaluation of evidence-based treatment programs for patients.

We are looking for forward thinking, team-orientated Assistant Practitioner to join our friendly and supportive team. You will be working alongside other physiotherapists, occupational therapists and Rehabilitation support workers/ Therapy assistants. In the Community Rehabilitation Team / Urgent Care team you would be working with patients in their own homes who present with a variety of long term conditions including complex neurology and falls.

We are passionate about patient care and together we work towards to maximising patient’s independence and providing quality of life through assessment, rehabilitation, preventing hospital admissions, and supporting the hospital discharge process.

This is an excellent and unique opportunity to develop your skills within the community setting. We would welcome applications from those with a desire to join a friendly, welcoming, and passionate multidisciplinary team.
