Surrey Heartlands ICS is looking for a Senior Project Manager to support the roll out of Population Health Management techniques (PHM across the system.

Population Health is an approach that aims to improve physical and mental health outcomes, promote wellbeing and reduce health inequalities across an entire population. This includes focusing on the wider determinants of health – which have a significant impact as only 20% of a person’s health outcomes are attributed to the ability to access good quality health care – and the crucial role of communities and local people. It is Surrey Heartland’s vision to become a thriving population health management system within 5 years and the journey has just started with the system being successful in gaining support from NHSE in becoming a Wave 2 system of the national population health management programme. This journey has also started with the development of the Population Health strategy and operating plan which has mapped out our journey to becoming a thriving population health system. A senior project manager is needed to support the Programme Leads to deliver the roll out of PHM as well embedding the operating plan and strategy across the system at all levels.

This role would be an ideal secondment opportunity, we currently have funding for 12 months.
