gtd healthcare is looking for an Advanced Clinical Practitioner to work at Elmwood Medical Centre in Buxton on a permanent contract with either a full time or part time position considered between the core practice hours of 8am - 6.30pm. Flexible working considered along with option of portfolio working which can attract additional enhancements to salary for out of hours working. Remote working considered for some aspects of role.

The job will involve working alongside a multidisciplinary team, including GPs, Nurse Practitioners, Clinical Pharmacist and Nurses to provide care and treatment to patients with either long term conditions, acute illness or undiagnosed undifferentiated diagnosis.

The post holder will be supported by the Clinical Lead and have regular 121's and appraisals with the Primary care Matron and practice manager.

You will be involved in Practice & clinical meetings and have the ability to influence change and improve care for patients as well as the opportunity to lead on specific clinical based projects within the Practice team.

The role involves a generous CPD entitlement to ensure development in Practice and also the opportunity for portfolio work.

Elmwood Medical Centre is situated in the picturesque town of Buxton in the High Peak within a comfortable 45 minute commute from Cheshire, Stockport, Tameside & Glossop and high peak areas and within 60 minute commute from Manchester and Sheffield areas.

The practice is located opposite the beautiful Pavilion Gardens and offers free on-site staff parking.

Elmwood Medical Centre has over 8000 patients and is part of the High Peak PCN which consists of 8 Practices working together to provide services to over 60,000 patients across the area. Through the PCN the Practice has access to clinical pharmacists and social prescribers with mental health support workers hopefully coming on board in the near future.

Currently, we deliver healthcare services to 2.4 million patients via GP practices for registered patients, walk-in centres, urgent care centres, clinical triage, acute home visiting and Alternative to Transfer services.
