An exciting opportunity has arisen to join our Long Term Conditions Support Services working within the Community Respiratory Team.  This role will provide clinical care for patients referred for assessment of their respiratory condition.  Patient assessment and care will be delivered in clinics and home visits. Service evaluation and development will be integral to the role.

We are looking for a Band 6 Respiratory Nurse to join our existing team of experienced nursing staff, a physiotherapist, rehabilitation assistants and an administrator.  You will need to be able to work independently, making autonomous clinical decisions and appropriate referrals to other services.  You will be required to undertake specialist clinical skills after appropriate training.  You will support delivery of pulmonary rehabilitation, including supervising classes of up to 12 respiratory patients, with support from other team members.

Central to this role are:

- strong communication skills

- expertise and experience in provision of care for patients with respiratory conditions

- experience of service development and evaluation

You must be a licensed car driver and have access to a car for this post.
