We are looking to recruit a highly motivated, passionate and enthusiastic Advanced Clinical Practitioner to work in the North West Locality of Bristol. We currently have a part time role (30 hours), Band 7 depending on relevant skills and qualifications.
The purpose of this role is to work autonomously as a specialist practitioner to identify and case manage patients with highly complex needs who require intensive community support, within their GP Primary Care Networks in partnership with the Integrated Network Team (INT) and the primary care team to prevent avoidable admissions and facilitate timely discharges from hospital into the community setting.

You will need to be a reliable, adaptable and self-motivated individual that is able to work as a good team player, have excellent communication, interpersonal skills and experience in autonomous practice.

As a specialist practitioner you will provide a service that focuses on achieving and demonstrating overall improvements in health care for patients who are frail and have several long-term conditions. You will also provide clinical leadership and direction to the INT, supporting the Locality Manager and Assistant Locality Manager to deliver excellent clinical outcomes. To provide organisational and service resilience, you may be expected to cross cover other teams or services, both within the locality and across Sirona services.

We currently have an established friendly and supportive team of Advanced Clinical Practitioners with a Clinical lead and a robust career development pathway.

• Registered Adult Nurse NMC level 1 or equivalent
• Must hold a degree level qualification
• Excellent interpersonal skills, able to build strong relationships with stakeholders/GP’s
• Demonstrate; an advanced level of physical examination skills, proficient clinical reasoning, the ability to interpret diagnostic tests where appropriate.
• Be able to diagnose patients who present with undifferentiated acute injuries and plan and implement any further care the patients may need
• Have a good working knowledge of safeguarding issues for adults
• Proficient Long Term Conditions/Urgent care management skills.
• Ensure that accurate, contemporaneous documentation is achieved
• Be accountable for care using research based knowledge to inform practice
• Non-medical prescribing qualification, registered with NMC, or prepared to work towards

This post is offered on a permanent basis for 37.5 hours per week.
