We are looking for an experienced, passionate, enthusiastic Band 6 registered Nurse or Occupational Therapist ready for the next step in their career working in In-Reach into Care Homes and the Memory Assessment Service pathways.

This is a unique experience to join a service focused on transformation, personal development and the provision of excellent care.

 

As a Trainee Advanced Clinical Practitioner working with In-Reach into Care Homes and Memory Assessment this is an exciting opportunity to develop knowledge and skills and building on experience, to enable the post- holder to gain a substantive ACP/ANP role on completion of following an agreed period of training, supervision and assessment of competence.
