In addition to transplantation, Royal Papworth Hospital has the UK’s largest Respiratory Support and Sleep Centre (RSSC) and is one of five UK centres providing Extra Corporeal Membrane Oxygenation (ECMO) to patients experiencing severe respiratory failure. It is also the only centre in the UK for a number of specialist services including Pulmonary Endarterectomy and Balloon Pulmonary Angioplasty (BPA).

We are the country’s main heart and lung transplantation centre with an international reputation for innovation and research excellence.

An exciting job share opportunity has arisen for a registered HCPC, NMC or Social Work England practitioner to join us as a Discharge Planning Specialist. You will work with your job share partner to play an integral role in the management of patient flow throughout the trust and facilitation of complex discharges within Royal Papworth Hospital. We are looking for an enthusiastic, proactive and knowledgeable clinical specialist to work with the existing post holder as a job share.

Applications must have a current clinical registration (NMC, HCPC or Social Work England) and will ideally have had some experience of discharging patients with complex needs.

Duties will include:

  • providing clinical expertise and support to the MDT especially in relation to patient’s suitability for repatriation or rehabilitation.
  • Lead on the process for proactively identifying patients who will have complex discharge needs prior to admission to hospital.
  • Case working individual patients requiring complex discharge planning pathway 3.
  • Lead on and support the multi-disciplinary team in resolving complex patient discharge problems.
  • Collaborative working with the patient flow team and the discharge planning team.
