Job overview:
Part time and full time (2 Clinical Pharmacists) required for BARDOC Limited.

Duties include medicines reconciliation on transfer of care and implementation of systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the BARDOC Family of Practices.

One role at Whitworth Medical Centre and other at Bury, Rock Healthcare.

Main duties of the job:
The post holder will provide clinical leadership around medicines optimisation, recall and review of medication and ongoing quality improvement.  They will manage some aspects of the quality and outcomes framework and enhanced services, with some focus on long term conditions, end of life care and care home residents as well as day to day acute medication guidance and support.
The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice

BARDOC's Family of Practices consist of five GP practices; Family Practices (Strand and Bowness Road), Rock Healthcare (Bury and Radcliffe branch), Whitworth Medical Centre and Knowsley Medical Centre (Bury). We are a close knit group of practices who work effectively together to deliver services which provide an enhanced level of care for our patients.

Clinical Pharmacy is a developing role within General Practice and this is a wonderful opportunity for those who are passionate about demonstrating how effective and impactful this will be.

Whitworth Medical Centre is the day to day location of the first role, with a requirement to work closely with the Team at the practice in this lovely village just a few minutes drive from Rochdale. Hours are flexible to meet the needs of both the practice teams and the successful candidate.

For the second role, you will be based out of BARDOC's main offices in Bury at The Rock Healthcare practice and also in their branch practice at Radcliffe Primary Care Centre. Hours, again, are flexible to meet the needs of Rock Healthcare's busy team and also to fit around individual needs of the successfulcandidate.

Job Description summary:

Primary Care Networks (PCN) have provided an exciting opportunity to develop and extend the role of the Clinical Pharmacist into the work of General Practice. BARDOC already boasts a great team of Clinical Pharmacists working throughout the five practices and beyond. We are looking to add additional Clinical Pharmacy support to enable a presence in all the surgeries.As such, you will be part of a broader, supportive and evolving team of pharmacists.

Salary:Negotiable depending on experience, knowledge and ability

You will receive a generous holiday allowance and contractual T&Cs and you will have access through auto-enrolment to the NHS Pension scheme.
