Heywood, Middleton & Rochdale is a dynamic and forward-thinking Boroughand we are looking to recruit a qualified and experienced nurse to join our established NHS Continuing Healthcare and complex cases team.

The postholder will ideally be a registered RNLD with significant knowledge and experience of the Transforming care agenda. The successful applicant will be expected to deliver a lead coordination and case management role for people with a learning disability and/or autism and lead on the implementation and coordination of the Care education and treatment review process (CeTR) and the Dynamic support database. You will also act as a point of contact for advice to other professionals and patients and/or their representatives.

The post holder will also contribute to the development of local strategies and work with wider members of the joint commissioning team and Greater Manchester colleagues to achieve national and local objectives.

HMR CCG and Rochdale Borough Council are at the forefront of integration with a pooled fund to support the Rochdale borough.

The post is a permanent part time position, 22.5 hours per week over 3 days.
