The post holder will work as an active member of the Home Treatment Team within the Older People’s Mental Health services in the Locality.

The post holder will take responsibility for the assessment and implementation of intensive care and treatment in the home of people with mental health needs, as well as people with dementia.

The post holder is a highly skilled professional practitioner with experience and specialist knowledge of the care of older people with mental health problems and mental health crises. They will be employed by NHSP for Berkshire Healthcare NHS Foundation Trust as the provider of mental health services in Berkshire.

Location/Base: Upton Hospital, Slough

Length of Placement:Starting ASAP until 30/10/2021

Shifts/Hours: Ranging over 7 days, hours can vary between either 09:00 to 17:00 or 11:00 to 19:00.

Band 7 Rate -

Day Rate - £23.90 per hour

Night/Sat Rate - £30.13 per hour

Sun/Bank Holiday Rate - £37.55 per hour

Length of Role: Starting ASAP until the 30th October 2021

RESPONSIBILITIES

2.1 CLINICAL

 

  • To manage and be responsible for the mental health care of an allocated caseload of patients.
  • To undertake comprehensive initial assessments in a range of settings following acceptance of referral, taking into account the persons physical, psychological, social and spiritual needs.
  • To develop and implement an agreed plan of care with the service user and where appropriate, include the views of families and carers.
  • To provide a fast and flexible response to care needs, responding to referrals in a timely manner and providing intensive treatment (up to four times daily).
  • To identify and select from a range of health, social and voluntary agencies which could assist and improve the care of individuals and their families.
  • To initiate and carry out the agreed treatment plan
  • To monitor and evaluate the agreed care plan, making adjustments as appropriate in collaboration with the wider team by reporting in review or CPA meetings.
  • To have knowledge of a variety of therapeutic interventions including short term problem solving, crisis intervention and basic CBT techniques
  • To develop expertise in one specific area of therapeutic skills.
  • To develop and maintain effective liaison and communication with members of the Primary Care Team, Local Authority, Voluntary organisations on matters relating to patient care.
  • To participate actively in Service Development to enhance the nursing contribution and quality of care
  • To develop and lead relevant education programmes for formal and informal carers as a means of enhancing standards of care in the locality.
  • To provide specialist consultation and advice on issues relating to the care of older people with mental health problems to other professionals within the wider care community such as residential homes, extra care housing, day centres etc.
  • Ensure the correct application of CPA and Mental Health Clinical Risk policies and procedures.
  • To assist with triage of new referrals to the team and to signpost as required.
