An exciting opportunity is now available to appoint an enthusiastic and pro-active Discharge Facilitator (Physiotherapist) with the Leeds Integrated Discharge Service (LIDS), based within Leeds Teaching Hospitals Trust. This post is available as either a permanent post or 12 month secondment opportunity.

What is the role?

Leeds Integrated Discharge Service aims to prevent avoidable hospital admissions and works closely with the multi-disciplinary team, providing expertise in discharge planning and ensuring a seamless transition between hospital and community services.

LIDS is a multi-disciplinary team of Discharge Facilitators (Registered Nurses, Physiotherapi sts and Occupational Therapists). The team is managed and lead by the Clinical Service Manager, supported by the Operational Lead and Clinical Educator.

As a Discharge Facilitator you will be responsible for the assessment of patients identifying any support needs on discharge and co-ordinating and communicating their discharge plans.

You will take a lead role in the daily MDT, working closely with other professionals and providing expertise in discharge planning.

About you:

Are you a Registered Physiotherapist with experience in discharge planning?


Do you have experience of working with a wide range of professionals including medical, nursing, Allied Health Professionals, Adult Social Care and the third sector?

Are you an enthusiastic, well-motivated individual with excellent communication skills and the ability to organise and prioritise own and other's workload?

Are you passionate about providing high quality patient centred care?

Would you like to be part of a dynamic, fast paced, forward thinking team?
