Occupational Therapy at the forefront of change in Leeds Community Healthcare

Following a successful pilot based on the principles of "Home First" and "Discharge to Assess" we are excited to offer Band 6 Occupational Therapy positions in the Therapy Supported Discharge project. By joining our team you will be a part of a pioneering project shaping the future of Therapy Services within Leeds.

Therapy supported Discharge in an Occupational Therapy led service aiming to achieve fluid working between the Acute and Community Trust. Our aim in doing this is to improve patient flow, facilitate timely discharge and improve patient's outcomes by maximizing their opportunities to live a fulfilling life after leaving hospital. This is a unique opportunity to be involved in initiatives to build a more comprehensive rehabilitation service across the Trust. You will be leading the way! We respect and encourage our Band 6's to take on a shared responsibility with service development. This is a fantastic opportunity for motivated and innovative Occupational Therapists who want to develop their leadership skills and gain experience in a wide range of clinical specialties.

In turn we are looking for enthusiastic Occupational Therapists who are interested in new ways of working. Experience in community and/or acute working is desirable. Experience should be underpinned by sound clinical knowledge, in evidence based assessment and management of a variety of conditions in the adult lifespan. Positive risk taking is a key element to practice, we are seeking staff that has well developed strategies for managing risks. You should possess excellent communication skills and be able to take your own initiative and work autonomously to drive forward rehabilitation.

Due to the nature of the role it is essential applicants are able to drive and a car suitable for business purposes.

If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost from your salary. There will be no charge if you are enrolled in the DBS Update Service.
