An exciting opportunity has arisen for a Community Staff Nurse to play a key role in providing a range of nursing skills to patients with a variety of complex and non-complex needs, both proactively and responsively, to prevent hospital admission.

Primary Care Network (PCN) community teams are multidisciplinary teams responsible for coordinating care delivery to patients on the caseload. The delivery of high quality care is organised around an individual’s needs, identified through holistic assessment and referral centred around collaborative working by a range of service providers.

The successful candidate will be responsible for working without direct supervision in a number of community settings, following the nursing process and delivering appropriate nursing care. They will also act as an information resource for patients and colleagues, and work collaboratively with our health and social care partners. They should possess a wide range of clinical competencies including but not limited to wound care, catheter management, palliative care, phlebotomy, administration of medications/injections, and compression bandaging. Development opportunities may be available for the right candidate once in post if further competencies are required.
