Norfolk Community Health & Care have an exciting opportunity available for a Band 5 Nurse to join our Triage Team in the Norwich Locality Hub.

The role involves the initial and on occasions the enhanced triage of clinical work within at times a high pressured HUB environment to both, enhance the care provided and also to promote self-care through coaching and effective communication.

80% of your contracted hours will be within the Hub environment with the remaining 20% delivering care for patients in the community. A variety of skills and experience are required to promote high quality care across all services that the Hub work with as well as supporting community nurses.

You will work as part of the multi-disciplinary team delivering quality, timely and responsive care for patients in the community over 7 days a week.
