A Community Mental Health Practitioner will provide community based care to clients with complex mental health needs who have been referred into the integrated Community Mental Health Team. You will work as an autonomous practitioner managing and prioritizing individual caseload and will act as a Care Co-ordinator within the Care Programme Approach. The post holder will be responsible for the assessment of care needs and the development, implementation, and evaluation of programmes of care for a defined caseload of people with complex mental health and/or social care needs.

Providing high standards of skilled mental health care to individuals and their families within a defined geographical area., the role will include active participation in the service planning and development of the nursing and multi-disciplinary teams.

You will provide clinical supervision and support to junior members of the team and actively promote the values and policies of the service and the Trust. Community Mental Health Practitioner are active members of the multi disciplinary team and will work in close co-operation with secondary and primary care based professionals, other community agencies such as Housing, Probation, Police, Children Schools & Families and other Voluntary and Third Sector Agencies.
