We are seeking to recruit a Community Nurse Practitioner to provide and support the provision of high-quality patient care. This is an exciting opportunity for an experienced and dynamic practitioner to develop who is enthusiastic and motivated and willing to develop new ways of working. They will be integral in delivering Community Services in different ways across the Localities. Referrals are received from multiple sources including General Practitioners, West Midlands Ambulance Service, Care Homes, Community Nursing Teams and Therapy services. They will facilitate a multi-professional approach to service development and delivery to maintain and support patients within their own home and care homes promoting independence.

As a Community Nurse Practitioner, you will be responsible for the assessment of health and clinical needs of the patient, to undertake consultations with patients as an autonomous practitioner using advanced clinical practice skills, own clinical judgement to diagnose, treat; to prescribe where required, refer to other services and discharge patients.

A key part of the role will include making rapid clinical assessments of patients with undifferentiated conditions, making evidenced based treatment plans whilst aiming to ensure patients are cared for in the most appropriate place. You will be taking part of delivering Enhanced health in care homes, service requirements in respect of that care home in accordance with DES specification.

You will also be required to ensure delivery of high quality patient centered care measured by clinical effectiveness, patient safety and patient experience as well as meeting the key performance measures as required by the Trust.

This is an exciting role for the right candidate. You will be joining a developing and progressive service, giving you the opportunity to participate in the ongoing development of the team and the Clinical HUB. The successful candidates must have experience of service development and clinical leadership to ensure patients and their families receive timely, effective and supportive care. You will be managed and supported on a daily basis by the HUB lead and Medical Lead as well as giving support to Junior staff within the Dudley Clinical HUB.
