We currently have a one year fixed term post for a dynamic and forward-thinking Community Staff Nurse to join the Havens Primary Care Network (PCN) Community Nursing team.

The team is based in Peacehaven, and covers Peacehaven and Newhaven.

Our team works closely with our linked GP partners, social services, specialist nurses and local hospice to deliver excellent care in the community. We are well supported by a Trust which understands community nursing and delivers adult nursing care to patients across East and West Sussex.

Your responsibilities will be:

  • Visiting patients in their own homes and providing holistic, evidence-based nursing care. This will usually mean working alone, although you will have great support from a well-established and experienced team of nurses.
  • Carrying out nursing interventions such as wound care, medicine administration, diabetic management, IV drug administration, end of life care and syringe drivers, along with many other nursing interventions provided by the Community Nursing team.
  • Ensuring that you robustly document all interventions and relevant information on our electronic patient record system (SystmOne).
  • Supporting the caseload managers in ensuring the smooth running of the office and the team.

You must be able to travel around the Trust in a timely manner, so you must have a full driving licence.
