This is a pivotal role for an expert clinical nursing practitioner who can offer clinical leadership to nursing teams across Responsive Services, area-wide - primarily supporting admission avoidance and complex supported discharges pathways in the community. The Clinical Nurse Lead role will form part of an experienced group of fellow expert practitioners including Lead Occupational Therapists and Lead Physiotherapists working as a team across the area-wide Responsive Services of our Central Locality (Crawley, Horsham and Mid Sussex).

The post holder will provide proactive nursing and professional leadership, requiring a dynamic and forward-thinking nurse who, in addition to being an excellent communicator and mentor will also be able to support a team of Healthcare Assistants and Nurses. We are looking for the right candidate who can aim to support the service, the staff and the service users through complex nursing care pathways, which can include chronic health conditions and end of life pathways as well as ongoing therapy and rehabilitation. We are looking for a nurse who can lead by example, develop others, question in a constructive manner, be reflective, and instil confidence in others - a Clinical Nurse Lead who can develop and maintain nursing care which is safe, effective, responsive, caring and well led across all domains.

The role will require the right candidate to work closely with Clinical Service Managers, Team Leads, and the Senior Physiotherapists and Occupational Therapists, providing an expert person-centred service to adults in the wider community of central Sussex.
