Adult Community Health Services, Joint Emergency Team

Full Time 37.5 Hours per week: Rota to cover 08:00 am to 20.30 pm seven days a week

An exciting opportunity has arisen and we are looking to appoint a committed, highly motivated, enthusiastic and skilled Band 7 Nurse Lead to join our, dynamic, integrated multi- disciplinary team. This post is for Registered Nurse who is able to demonstrate advanced clinical skills, including physical assessment and clinical decision making for a range of common acute conditions. Central to this role is the need for the post holder to have a non- medical prescriber qualification and be able to actively prescribe.

The successful applicant will be self- directing and innovative with an understanding and passion for multidisciplinary working taking a lead role in the further development of collaborative working between Oxleas NHS Foundation Trust, Royal Borough of Greenwich and Lewisham and Greenwich NHS Trust.

The Joint Emergency Team is a multi-professional team consisting of Nurses, Occupational Therapists, Physiotherapists and Social Workers. The team provides timely intervention and support to people in their own homes providing crisis management and support during periods of acute illness thereby preventing unnecessary hospital admission. The role involves assessment, formulating differential diagnoses and collaborative working with a range of multidisciplinary teams to manage health and social care issues.
