The post holder will be responsible for the leadership, management and development of staff providing rehabilitation services under the banner of Intermediate Care which is inclusive of Rapid Response, Reablement/D2A and home based rehabilitation, working closely with the Head of Integrated Services & Community Therapies.

The post holder will be expected to work collaboratively across traditional Health and Social Care boundaries, identifying opportunities and driving a culture of local integration which puts the patient at the centre of service delivery.

In particular you will work closely with the Intermediate Care Matron, team leads for therapies and the locality leads for nursing in supporting patients care needs and handover following an admission avoidance episode of care, or following a hospital discharge.

The post holder will have overall responsibility for the management and/ or leadership of a range of staff including Occupational Therapists, Physiotherapists, Therapy Assistants and administrators.

The post holder will recognise the importance of systems working and will have a lead role in reducing delayed transfers of care into the community services and keeping patients at home.

Current employees should be aware that this position is advertised as a GWH position and appointment ends any TUPE protection.

All candidates are required to pay for their own DBS check where applicable. Standard DBS check costs £26 and will be recovered from your first pay; Enhanced DBS check costs £44 and will be split and recovered over the first 2 pay.

Bank posts requiring a DBS check; payment must be made prior to commencing.

Apprentices are exempt from payment.
