We are offering an exciting opportunity for a full time, Band 3 Health Care Support Worker to join the Older Persons Community Mental Health Team.

The successful applicant will be working across Portsmouth City, and is currently based at St James Hospital, though soon to be relocated to St Marys Community Health Campus. The role will focus on providing care and support to the older population of Portsmouth with mental health needs, including patients with a range of mental health diagnoses including Dementia and / or functional illnesses.

The successful applicant will be working as part of a dynamic multi-disciplinary team and will hold a caseload of service users, providing physical and psychological care to patients as part of a specified plan of clinical care, overseen by an allocated care coordinator or lead professional.

The applicant will require excellent interpersonal skills to liaise with service users, carers and the wider multi-disciplinary team. Experience of working in older persons mental health with functional and organic illnesses is desirable though not essential. It is important to have excellent time management skills and be able to work autonomously as well as part of a team.

The team are currently in the process of developing a dementia care pathway for Portsmouth and will be joining with Solent Remind in order to deliver this, therefore expanding our team and services we offer. We are keen to recruit an enthusiastic and forward thinking individual to develop our workforce and enhance the interventions available to our patients.

Supervision and support will come from the CMHT team leader or a clinician of a higher band; training and development opportunities are available.
