An exciting opportunity has arisen to join Solent NHS Trust as a Diabetes Specialist Nurse within the Community Diabetes Specialist Team. The team serves people living with diabetes and those caring for people living with diabetes within Southampton City. If you have passion and experience in diabetes care and are committed to making a difference to those living with diabetes, then we would welcome your application.

The team currently provide specialist advice and guidance through:

  • Multi-Disciplinary Team clinics
  • Structured education
  • Refresher education
  • 1 to 1 appointments
  • Home visits
  • Telephone support and advice
  • Access to the FreStyle Libre
  • Bolus advice meters
  • Bespoke education packages
  • Support to Primary Care and health care professionals, and much more

Our team comprises of one Consultant, five Diabetes Specialist Nurses, dietitians and three administrators. We are also lucky to have the Steps to Wellbeing team working alongside us.

We are looking for a Diabetes Specialist Nurse to work with us for 15 hours per week, which could be performed alongside another part time role you may already be involved with.
