Do you have the drive and skills to improve pharmacy services for patient’s at the most acute stage of their hospital admission?

If so, we have an exciting opportunity for you to join the Care Homes Pharmacy Team and contribute to the development of the Medicines Optimisation in Care Homes Service for Leicester Leicestershire and Rutland (LLR) working closely and under the leadership of the Advanced Specialist Pharmacist for Care Homes.

The benefits of a clinical pharmacy led medicines optimisation service for care home residents is evidenced well and now supported nationally through the pharmacy integration fund.

We have an opportunity for a self-motivated, experienced and dynamic Pharmacy Technician to join the Medicines Optimisation in Care Homes Service and develop new and existing pharmacy services for Care Homes across Leicester Leicestershire and Rutland Leicestershire and Rutland. (LLR) under the leadership of the Advanced Specialist Pharmacist for Care homes and the existing Care Homes Teams in Primary Care

The post holder will work in and across a number of settings in LLR to maximise the potential of Medicines Optimisation and innovate in supporting the improvement and integration of pharmacy services provided to Care Homes:

  • In Care Homes with patients, family members, carer’s and Care Home staff.
  • With GP practices responsible for patient care
  • With other primary care health and social care service providers
  • Across the interface of Primary and secondary care
  • In the acute stages of hospital admissions
  • At discharge from or admission to care settings.

University Hospitals of Leicester has one of the busiest Emergency Departments in the country with one ED serving a population of over 1 million people. Our acute medicine department also has a large Acute Medical Unit, an Acute Ambulatory Care service, a Short Stay Unit, an Acute Care Medical HDU and 2 Acute Frailty Units; which includes the newly formed Frailty Emergency Squad (FES).

Building on your broad pharmacy experience you will provide pharmacy services training and advice to an identified number of Care Homes across LLR and develop close links with the UHL Frailty Team to ensure a high level of medicines optimisation and review, using nationally recognised tools.

  • identify areas for improvement in current practice
  • work with teams across the interface expanding the role of the pharmacy team in primary and secondary care to optimise medication for residents of Care Homes.
